New Horizons in Hypertension Management: 2025 AHA/ACC Guidelines

The 2025 AHA/ACC guidelines provide updated recommendations to help clinicians identify, manage, and treat high blood pressure more effectively. Key Updates in the 2025 Guidelines

1. Blood Pressure Classification & Early Intervention
The 2025 guidelines maintain the 2017 thresholds but emphasize earlier intervention for patients at elevated cardiovascular risk:

  • Normal: <120/80 mm Hg
  • Elevated: 120–129/<80 mm Hg
  • Stage 1: 130–139/80–89 mm Hg
  • Stage 2: ≥140/90 mm Hg

Patients with a history of CVD, CKD, or diabetes, or a 10-year ASCVD risk ≥7.5%, should be considered for early treatment—even at Stage 1.

2. Pharmacologic Treatment

  • Stage 2 hypertension: initiate two-drug therapy, ideally a fixed-dose combination.
  • First-line options: ACE inhibitors, ARBs, calcium channel blockers, and thiazide diuretics.
  • Stage 1 with elevated risk: monotherapy may be sufficient, escalating as needed.

3. Cognitive Health & Blood Pressure Targets
Control of hypertension is crucial for preserving cognitive function. Adults with hypertension should aim for <130/80 mm Hg to reduce the risk of dementia and cognitive decline.

4. Hypertension in Pregnancy

  • Monitor BP before, during, and after pregnancy.
  • Treat chronic hypertension at ≥140/90 mm Hg.
  • Consider low-dose aspirin in high-risk patients to prevent preeclampsia.

5. Lifestyle & Home Monitoring
Lifestyle remains foundational: DASH diet, reduced sodium (<2,300 mg/day, ideally 1,500 mg), alcohol moderation, regular exercise, and stress management. Home BP monitoring improves adherence and allows for more accurate treatment adjustments.
