Value Based Care: Improving Chronic Disease Care
Apart from providing CME education to individual providers, we offer clinics a structured program to improve quality of care of various chronic diseases. Currently, we offer a program to improve the management of heart failure. Why heart failure? Because there is a large gap between Guideline-Directed Medical Therapy (GDMT) vs the actual practice of medicine:

The Data:
- 82% are on ACE/ARB, 81% on beta‑blockers
- Only 23% receive MRAs, and just 3.6% get SGLT2 inhibitors
- Less than 1% achieve target doses across all three traditional GDMT classes (ACE/ARB, BB, MRA), much less all four including SGLT2i
This under-treatment drives avoidable hospitalizations, increased mortality, and inflated healthcare costs.
